Definition:

Disease management is a system of coordinated heath care interventions and communications for defined patient populations with conditions where self-care efforts can be implemented. Disease management empowers individuals, working with other health care providers to manage their disease and prevent complications.

It is also the process of reducing healthcare costs and/or improving quality of life for individuals by preventing or minimizing the effects of a disease, usually a chronic condition, through integrated treatment. The common goal is to enhance clinical outcomes by developing and implementing methods and technologies that result in symptom abatement and the reduction of the number, intensity, and duration of needed service interventions.
